Concentration % (w/v) = (Mass of Solute in grams ÷ Volume of Solution in mL) × 100. Weight-Based Dose = Weight (kg) × Prescribed Dose (mg/kg). Dose Adjustment % = ((New Dose − Standard Dose) ÷ Standard Dose) × 100.
